Perimitral atrial flutter associated with a protected coronary sinus after a Maze IV procedure and concomitant mitral annulus repair

چکیده
Introduction The Maze IV procedure during cardiac surgery is an established treatment for patients with atrial fibrillation (AF) concomitant with a mitral valve repair or replacement. Recurrent atrial tachyarrhythmias are mainly left atrial (LA) flutter, which is associated with gaps on previous surgical lesions. Herein, we report a case in which a 3-dimensional mapping system could demonstrate a typical perimitral atrial flutter but radiofrequency (RF) ablation of a gap on a previous lesion could not terminate the tachycardia.

Interatrial electrical dissociation after catheter-based ablation for atrial fibrillation and flutter.
A 58-year-old man with hypertension and diabetes with recurrent persistent symptomatic atrial fibrillation resistant to sotalol-facilitated cardioversions was referred for ablation. Catheter-based radiofrequency ablation for persistent atrial fibrillation was performed, which consisted of isolation of all 4 pulmonic veins; linear ablations along the left atrial roof; an ablation line from the l...
متن کاملCatheter Ablation of the Superolateral Mitral Isthmus Line: A Novel Approach to Reduce the Need for Epicardial Ablation.
BACKGROUND The mitral isthmus is a critical part of perimitral reentrant tachycardia, as well as an important substrate of persistent atrial fibrillation. Deployment of an endocardial mitral isthmus line (MIL) with the end point of bidirectional block may be challenging and often requires additional epicardial ablation within the coronary sinus. METHODS AND RESULTS The study population compri...
متن کاملCatheter Ablation of the Superolateral Mitral Isthmus Line
METHODS AND RESULTS: The study population comprised 114 patients with perimitral flutter who underwent de novo ablation of an MIL. The initial 57 patients (group A) underwent catheter ablation using a novel superolateral MIL design, connecting the left-sided pulmonary veins with the mitral annulus along the posterior base of the left atrial appendage visualized by selective angiography. The nex...
متن کاملMarshall bundle reentrant atrial tachycardia after the Cox-Maze IV procedure: The last barrier of the conduction pathway between the coronary sinus and left atrium
A 74-year-old woman who developed atrial tachycardia following the Cox-Maze IV procedure underwent catheter ablation. The reentrant circuit included the coronary sinus (CS), Marshall bundle (MB), distal MB-left atrial (LA) connection, and anterolateral mitral annulus. The distal MB-LA connection was the last barrier in the conduction pathway between the CS and the left atrium.
متن کاملBipolar radiofrequency catheter ablation for refractory perimitral flutter: a case report
BACKGROUND Mitral isthmus is often targeted as a part of stepwise approach during radiofrequency ablation for persistent atrial ablation. Acute success rate in achieving the mitral isthmus block is only modest, late reconduction rate is relatively high and, consequently, incomplete lesion may be proarrhythmic. We describe the first-in-man experience with successful MI ablation by bipolar RF ene...
